Every dollar you deposit is available for use on Form’s services or skincare—no hidden fees, no points to collect.

  • Layaway-Style Savings: A set amount is automatically deposited into your Form Account every month.
  • Freedom to Choose: One hundred percent of what you deposit can be spent on any service or product you desire.
  • No Expiration: The Form Friends program never expires. If you decide to cancel after a year, simply provide 30 days’ notice and use any remaining balance on services or skincare.
  • Easy Setup: Select the program that fits your needs, complete an application, and authorize monthly payments.
  • Extra Perks: Enjoy discounts on treatments* and skincare products while you’re enrolled in Form Friends. Perks reset each year.
