The Form Friends Loyalty Program is a layaway program designed to make our treatments affordable. The program also allows us to thank you for choosing us by providing you with discounts, gifts, and complementary services.
Each month, your set amount will be automatically deposited into your Form Account. One hundred per cent of the money deposited can be used towards any Form service or product you desire. There are no hidden fees and no need to collect points.
Yes! As part of Form Friends, you will receive discounts on treatments and skincare products. These discounts apply as long as you are a part of the program. Perks reset every year.
The Form Friends program never expires. Try Form Friends for one year and if you’re not loving it, you can cancel with a 30 day notice. Any money remaining in your account will need to be used on Form skincare or services.
