End-User License Agreement – FMA QuickBooks Reporting Integration
This integration (the “App”) is an internal tool built and operated by Form Medical Aesthetics (“FMA”), 100 – 1721 Portage Ave, Winnipeg, MB R3J 0E4. It connects only to FMA’s own QuickBooks Online company, for internal financial reporting and analysis.
1. Licence. Use of the App is limited to FMA and its authorized management staff. It is not offered, sold or distributed to any other business or person.
2. Access. The App accesses QuickBooks Online data only after an FMA administrator authorizes it through Intuit’s OAuth process. That authorization can be revoked at any time from QuickBooks Online (Settings → Apps) or by disconnecting the App.
3. Data use. Data retrieved from QuickBooks Online is used only for FMA’s internal accounting, reporting and financial analysis. It is not sold, rented or shared with third parties for their own purposes.
4. No warranty. The App is provided “as is” for internal use, without warranty of any kind.
5. Limitation of liability. To the extent permitted by law, FMA is not liable for indirect or consequential damages arising from use of the App.
6. Governing law. This agreement is governed by the laws of Manitoba and the federal laws of Canada that apply there.
