Men’s Sexual Health Consult

Many factors can affect sexual health including physical health, mental health, and hormones. Many people are not aware of all the sexual health treatment options available.

At Form, we offer a men’s sexual health consultation to determine if you are a candidate for the F Shot. Options for addressing your concerns will be discussed. At your consultation you can expect Dr Lachman to review your medical history to see if you are a good candidate for the F shot. The cost of this consultation will be used towards your first treatment.
