Matt Lachman

Matthew Lachman brings a unique skillset to Form. He is passionate about client’s looking and feeling their best. Dr Lachman is the only provider of Venaseal in Manitoba. He has years of experience treating veins, from small spider veins, large varicosities, chronic ulcers and hand veins. He is a member of the International Association of Regenerative Therapy, the American College of Phlebology and of the Canadian Society of Phlebology. In addition to vein treatments he also treats a variety of conditions causing acute and chronic joint pain. Ultrasound imaging of joints is included with consultations. Treatment modalities include Platelet Rich Plasma treatments aka PRP and Prolotherapy. Dr Lachman also provides men’s sexual health treatments including the F Shot, fully embracing Form’s objective to have our clients look and feel their best in all aspects of their health.
